Payment & Reimbursements

Understanding Payment & Reimbursement

Cardiopulmonary Training Center

We believe in transparency. This handout explains how payment works, what reimbursement may be possible, and what we can and cannot do on your behalf.

Coverage and reimbursement are determined by your insurance provider. Cardiopulmonary Training Center does not guarantee reimbursement for any service.

How Our Services Are Paid

Cardiopulmonary Training Center operates primarily as a cash-pay service.
Payment is due at the time services are provided unless otherwise arranged in writing.

This approach allows us to:

  • Provide timely access to care
  • Offer flexible, community-based service
  • Avoid insurance delays and denial
  • Focus on clinical quality and patient education

Can I Get Reimbursed by My Insurance?

Possibly — but reimbursement is not guaranteed.

Some patients are able to receive partial or full reimbursement from their insurance plan when certain conditions are met, such as:

  • A physician referral or order
  • Medical necessity as defined by the payer
  • Coverage under your specific insurance plan

Each insurance plan is different. Coverage decisions are made solely by your insurer, not by Cardiopulmonary Training Center.

Services That May Have Reimbursement Potential

Depending on your plan, you may be able to seek reimbursement for:

  • Pulmonary Rehabilitation services delivered under physician referral
  • Pulmonary Function Testing (PFTs) performed under physician order
  • Certain respiratory education or supervised exercise services
  • Services covered under HSA, FSA, or employer wellness benefits

⚠️ Coverage varies by plan, diagnosis, setting, and payer policy.

Services Typically Not Covered by Insurance

The following services are usually not reimbursed and are considered private-pay:

  • Community or group education classes
  • Respiratory escort or accompaniment services
  • Concierge or private respiratory support
  • Travel-related respiratory assistance
  • Non-medical caregiver training

These services are offered because they provide meaningful value, even when insurance does not cover them.

What We Provide to Support Reimbursement Requests

While we do not bill insurance, we are happy to provide documentation that may help you submit a claim, including:

  • Detailed service receipts
  • Dates and duration of services
  • Diagnosis codes (when ordered by a physician)
  • Procedure code references (for informational purposes only)
  • Session summaries and progress notes

This documentation is sometimes referred to as a “superbill” or reimbursement support packet.

What We Do Not Do

To remain compliant and transparent:

  • We do not submit claims to Medicare, Medicaid, or commercial insurers
  • We do not guarantee reimbursement
  • We do not communicate with insurance companies on your behalf
  • We do not determine coverage or benefits

Your insurer makes all final coverage decisions.

Medicare Patients — Important Information

Medicare has specific rules about which services are covered and where they must be provided.

  • Some services (such as Medicare Pulmonary Rehabilitation) are only covered when delivered in hospital-based programs
  • Services provided at Cardiopulmonary Training Center may not meet Medicare’s coverage criteria, even if they are clinically appropriate

Medicare patients are welcome to participate in our services on a self-pay basis.

Tips to Improve Your Chances of Reimbursement

Before submitting a claim, consider:

  • Calling the number on the back of your insurance card
  • Asking if your plan covers the service under outpatient rehabilitation or diagnostic testing
  • Confirming whether a physician order is required
  • Asking about visit limits, deductibles, or pre-authorization

We recommend keeping copies of all receipts and documentation.
